Job Description:

SUMMARY

The Medical Record Clerk is responsible for managing patient health records, ensuring accuracy, confidentiality, and compliance with all legal and regulatory requirements.

 

PRINCIPAL ACTIVITIES & RESPONSIBILITIES:

  • Organizes, manages, and maintains physical and electronic medical records.
  • Retrieves patient charts for physicians and other authorized personnel.
  • Reviews records for completeness and accuracy, flagging any discrepancies.
  • Scans, files, and upload medical documents into electronic health record (EHR) systems.
  • Ensures records are stored securely and confidentiality is maintained at all times.
  • Responds to requests for medical records from patients, providers, and external agencies in compliance with HIPAA regulations.
  • Performs regular audits f records to ensure proper filing and documentation standards.
  • Assists with release of information (ROI) processes and authorization forms.
  • Collaborates with medical staff and administrative teams to support efficient workflow.
  • Performs other clerical duties such as data entry, faxing, and copying as needed.
  • Demonstrates professionalism in all interactions and maintains positive, respectful relationships with patients and tribal members.
  • Other duties as directed by management.

 

WORKING CONDITIONS & ENVIRONMENT:

  • Moderate noise level with frequent interruptions and distractions.
  • Office/clinical setting.

 

PHYSICAL & MENTAL DEMANDS:

  • Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
  • May require prolonged periods of sitting and computer use.
  • Vision abilities required by this job include close vision and the ability to adjust focus.
  • Must be required to push, pull, lift, and/or carry up to 25 pounds.

Experience and Skills:

MINIMUM JOB REQUIREMENTS:

  • Must be at least 18 years of age.
  • High School Diploma or GED required.
  • Previous experience in a medical office or health information department.
  • Familiarity with medical terminology and HIPAA regulations.
  • Proficiency with EHR systems and office software (e.g. Microsoft Office).
  • Attention to detail and accuracy.
  • Ability to handle sensitive information with discretion.
  • Excellent time-management capabilities and good written and oral communication skills.
  • Ability to communicate clearly and effectively in English, verbally, in writing or by other acceptable means.
  • This is a safety sensitive position and will be subject to pre-employment drug testing and criminal history background check, which will include fingerprinting.
  • Must have employment eligibility in the U.S.
  • Indian preference will be observed in the hiring process.
